Hi Cedrick, thanks for sharing your story with us. To start, maybe you can tell our readers some of your backstory.
My passion for chronic disease prevention began long before I founded ChronicCare. As a pharmacist, I witnessed firsthand the devastating impact of conditions such as diabetes, hypertension, and heart disease on patients and their families. My journey began at my home congregation, the North Greece Road Church of Christ. Each Sunday, I witnessed the profound impact of chronic disease on members of our community as they sought comfort, prayer, and support. This inspired me to dedicate my career to educating and empowering individuals through community seminars, webinars, health education programs, and patient coaching.
These efforts ultimately led me to write Self-Care Advocate: 7 Steps to Prevent Chronic Disease, a book designed to help people take a proactive approach to their health and reduce their risk of chronic illness through practical lifestyle and self-care strategies.
As I worked with patients, I realized that education alone was not enough. Many patients needed ongoing support to manage medications, coordinate care, and stay engaged in their treatment plans. This realization inspired the creation of ChronicCare.
Today, ChronicCare extends healthcare beyond the traditional office visit through a remote, pharmacist-led care coordination model. Using Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and other value-based care services, we provide continuous support from a distance, helping patients better manage chronic conditions while enabling healthcare providers to improve outcomes, reduce costs, and deliver more proactive care.
We all face challenges, but looking back would you describe it as a relatively smooth road?
No, it has not been a smooth road, but the challenges have made the journey even more meaningful. One of the biggest struggles was educating healthcare providers about the value of Chronic Care Management (CCM). While the benefits to patients are clear, many practices were unfamiliar with the program or lacked the resources to implement it effectively.
Another challenge was building a remote healthcare model that could seamlessly integrate with different electronic health record systems, workflows, and provider organizations. As a startup, we also faced the typical hurdles of securing partnerships, generating revenue, hiring qualified staff, and navigating the complexities of healthcare regulations and reimbursement.
Perhaps the greatest challenge has been changing mindsets. Our healthcare system has traditionally focused on treating illness after it occurs, while ChronicCare is built around prevention, proactive monitoring, and continuous engagement. Convincing organizations to embrace this shift takes time.
Despite these obstacles, every challenge reinforced our mission. Seeing patients gain better control of their health, reduce hospital visits, and feel supported between appointments reminds us why this work is so important and why the journey has been worth it.
We’ve been impressed with ChronicCare, but for folks who might not be as familiar, what can you share with them about what you do and what sets you apart from others?
ChronicCare is a pharmacist-led healthcare services company dedicated to improving outcomes for patients with chronic conditions through remote care coordination and value-based care programs. We partner with physician practices, health systems, community health centers, and other healthcare organizations to deliver Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Transitional Care Management (TCM), medication therapy management, and patient education services.

What I am most proud of is that our work aligns clinical excellence with a genuine commitment to prevention and patient empowerment. The foundation of our company is the belief that healthcare should not begin and end with an office visit. Can you talk to us a bit about happiness and what makes you happy?
What makes me happiest is knowing that I am living out my purpose by serving others and making a meaningful difference in their lives. Whether it’s helping a patient better manage a chronic condition, empowering someone to make healthier choices, mentoring an aspiring healthcare professional, or supporting a healthcare organization in improving patient outcomes, I find deep fulfillment in creating positive impact.
On a personal level, I value growth, family, and meaningful relationships. Some of my greatest joys come from spending time with loved ones, learning new things, and seeing the people around me succeed and thrive.
Spiritually, I believe that each of us has been given unique gifts and talents to serve others. For me, that is the truest expression of the Kingdom of God. I find fulfillment in using my knowledge, experiences, and resources to help people live healthier, more purposeful lives. For me, success is not measured solely by financial achievements or professional milestones, but by the lives touched, the communities strengthened, and the legacy of service we leave behind.
